The Florida Centre for Reading Research offers a series of teacher instruction resources, for Blending Sounds, Phoneme Blending, Segmenting Sounds and Writing Simple Words.  They also provide practice for children in Syllable Pattern Activity Sheets and Phoneme Segmenting and Blending Activity Resource Sheets.
